ROCS COMMUNITY CARES
The ROCS Community Cares program provides individuals with a compassionate and knowledgeable Community Cares Coordinator, who works closely to identify needs and connect clients and families to resources, such as food, housing, health insurance, health information, quality healthcare, and social services.

Through advocacy and education, we provide the assistance for clients to take the steps necessary to overcome their unique obstacles and achieve long-term stability and independence.
Who We Help

SD Medicaid patients

Individuals with health disparities (mental health, medical issues, obstacles to accessing/scheduling care)

Individuals struggling with chronic disease self-management or who are disabled (themselves or a household member)

Those with transportation or communication barriers

Individuals experiencing repeated hardships or homelessness
