Our CHWs provide wraparound support that addresses every aspect of a person's life. We advocate, educate, and walk alongside each individual on their path toward stability, independence, and improved well-being.

Who We Help

  • SD Medicaid patients

  • Individuals with health disparities (mental health, medical issues, obstacles to accessing/scheduling care)

  • Individuals struggling with chronic disease self-management or who are disabled (themselves or a household member)

  • Those with transportation or communication barriers

  • Individuals experiencing repeated hardships or homelessness

  • Health promotion and coaching (1-on-1 & workshops)

  • Meal planning and nutritional guidance

  • Scheduling medical appointments and transportation

  • 1-on-1 coaching tailored to individual needs and goals

  • Referrals to healthcare professionals

  • Assist with securing stable employment and overcoming personal challenges

  • Assist with rental applications, securing a lease, and addressing home safety concerns

  • Addresses out of home placement (residential, assisted living, treatment programs, etc.)
