SD CARES HOUSING PROGRAM

SDHDA and partner agencies are working to assist South Dakotans who have been impacted by COVID-19 and the related economic crisis by offering financial assistance to help stabilize their housing situation.

NOTICE -  the ERA program will end September 1, 2025

and HAF will end in 2026.

COMMUNITY CARES

PROGRAM

REFERRAL NEEDED TO APPLY

Community Cares provides referred individuals with a compassionate and knowledgeable Community Cares Coordinator, who works closely to identify needs and connect clients and families to resources, such as food, housing, health insurance, health information, quality healthcare, and social services. Through advocacy and education, CCC's provide the assistance for clients to take the steps necessary to overcome their unique obstacles and achieve long-term stability and independence. From navigating housing and transportation to scheduling medical appointments, this program serves as a link between health/social services and the community to improve service delivery quality and cultural sensitivity. When we strengthen an individual's ability to attain wellness and address social determinants of health, we can construct a stronger, healthier community.
